What formed

A breaker block is a failed order block: once price breaks through an order block, that same zone can flip roles, support becoming resistance or resistance becoming support, and act as a barrier when price returns to it.

It forms when an order block is decisively broken, then retested from the other side. The flip in role is what turns a spent order block into a breaker.
